NOTICE OF PRIVACY POLICY & PRACTICES
ONLINE PRIVACY POLICY SereniumScreen respects your privacy rights as an online visitor to the Site and user of our services and recognizes the importance of protecting any information collected about you. This Privacy Policy guides how the SereniumScreen collects, stores and uses online information provided by you to us on this Site only. This Privacy Policy is used to supplement the HIPAA mandated NPP set forth above. In the case of any conflict between this Privacy Policy and the NPP, the terms set forth in the NPP shall prevail.
What information is collected automatically? SereniumScreen may collect IP addresses (the Internet address of a computer) to track a user’s session. As the user starts the SereniumScreen survey and other services, he / she may provide additional information on the site (e.g. their email address, primary care physician and other administrative information). Prior to starting the SereniumScreen survey and other services, we may analyze this data for certain trends in statistics, such as how long users spend on the Site. We do not link your IP addresses to anything personally identifiable to you, prior to you starting the SereniumScreen survey and other services portions of the website. The uses, disclosures and your rights with respect to all personal information collected from you once you start the SereniumScreen survey and other services is covered by the NPP.
Is the information that I provide safe? SereniumScreen maintains appropriate administrative, technical and physical safeguards to ensure the confidentiality, availability and integrity of the information that you provide us as required by the HIPAA Security Rule and applicable state law.
Is my information shared with anyone without my knowledge or approval? SereniumScreen will provide an analysis of your SereniumScreen survey and other services if you approve this sharing, which could be by email delivery or secured link delivery with your primary care doctor, dentist or other healthcare provider. Such shared analyses may provide more technical medical data to them, than the consumer report being shared with you. In addition, if your doctor, dentist, other healthcare provider, insurer, employer or other sponsored groups initiates a SereniumScreen survey and other services with you while you are in their waiting room, online at home, on a mobile device, at work or through other vehicles, then the analyses from your voluntary submission of this information will be shared with you and also be shared with these parties, which could be by email delivery or secured link delivery. This information sharing is intended to improve your healthcare, as these parties will try to get more medical services to individuals who need additional healthcare options. These parties may or may not be funding the distribution of these SereniumScreen surveys and other services to you.
Will my data be used without my knowledge and approval? To improve healthcare outcomes and lower healthcare costs, SereniumScreen may do aggregate analyses and sub-analyses on SereniumScreen surveys and other data from users. Such analyses will be done on anonymous data sets and the individual information and privacy will be respected.
Do you use cookies? Cookies are small pieces of data that are sent from web servers to your browser and stored on your computer’s hard drive. Cookies cannot access other data on your hard drive and will not damage your computer in any way. The Site limits the use of cookies for only storing session state information. Session state information is gathered in the Site so that as you move from page to page within the Site, your interaction with the Site can be appropriately modified without the need of asking for personal information repeatedly. Session state information is only kept during a browser session and is automatically deleted by your browser at the end of every session.
What is your policy for collecting information from children? The Site is focused on providing wellness health services focused on sleep and respiratory conditions that benefits individuals including children. For children, the site encourages their parents or guardians to provide their information. Their parents can use the “Child Survey” to input the information for their children and their children do not need to access the site themselves. Parents can allow their older children to enter the site and complete information as an adult using the “Adult Survey” on their own. SereniumScreen cannot prevent people under the age of 18 from inputting information into the site by providing misleading demographic information or not following the site’s instructions. SereniumScreen does intend to collect any information from anyone under the age of 18 and takes appropriate steps to follow this policy. In addition, all data is maintained with appropriate security in accordance with HIPAA and other regulations.
